Coverage Options:

•Employee: $15,000 or $30,000; guarantee issue: $30,000.
•Spouse: 50% of employee’s election; guarantee issue: 50% of employee’s election.
•Dependent children to age 26:
$5,000; guarantee issue: $5,000.
Lump-sum benefit can be used to pay for the direct and indirect cost of a covered hospitalization, such as:

• Transportation
• Child Care
